I think it's been like that since at least 292. At least it showed up like that for me when I checked 292 out.

I had that problem as well.

I actually uploaded the attached image to correct it.

Although, from a fresh install of Beta 2, the same image appeared fine from the outset.

Image goes in roundcube/skins/default/images/buttons/ directory.

It's been like that a lot longer than 292... A fresh install of the SVN version at least a month ago had that error. Copying in the new file does the trick. The buggered one was a few bytes short of an icon (which I'm guessing is similar to being a few beers short of a six-pack - does the job, just not completely).
