Fix HTML for vacation plugin

It appears I may have the vacation plugin working, but when I click it to use it in Roundcube the HTML is screwed up.  Does anyone know how to fix this?

I've attached a screenshot.  I'm using Roundcube 1.1.2 or 1.1.3.

I am getting the same problem, for technical reasons I can't run the managesieve version of vacation responders, this one looked like a viable option except for the formatting.

There is another version of the same plugin here which seems to format better on larry
