upgrade from 0.2-beta odd login failure

Roundcube user for many versions. Always upgrade manually, but attempted to use the installer.

Installation passed, database updated, config file updated, etc.

When I attempt to login, roundcube keeps redirecting to a fresh login page without any error message.

I cannot login.

Thoughts?

I've created an entirely new, clean database, updated the db.inc.php file and also grabbed a fresh main.inc.php file.

Still have the same error. Login just jumps back to login!

Oops.  Simply forgot to replace new index.php file
