Duplicates in Database

using version 8.02 users are losing address books. Noticed in the DB the same user has 6 entries. Some have case differences some have different mail_host.  In the Identities table  the user has 6 entries.  In the contacts table the user has duplicate contacts that are linked to the different uses entries.

Also noticed that the login screen ask for user, password, and server.  The server field I was told just showed up 2 weeks ago and that is when the problems began.

1)How to fix the login screen?

2)How to get rid of the duplicates and get the DB cleaned up?

First your really should update version 0.8.2 is very out of date. To change the case behavior change login_lc in the config file. To set a default mail server set default_host in the config.
