Sinature and logo disappeared

Hi, strange stuff going on..all Settings look as as set, but now my  full signoff, with company logo, name, address, tels,  no longer shows..just has gone..will have to rebuild it unless I can understand if this a glitch and has a solution..Anyone else find this is happening ?

In the database are the multiple records in the users table for the same account?
