In IE8 the window doesn't open centered in the browser...is that a known issue?  Is there an easy fix?

Attached is a version which plays a little bit nicer with Roundcube v0.4.

Fixes:
#1- IE8 positioning bug
#2- contextmenu bug
#3- Better positioning of compose containers if taskbar is hidden
#4- Support for Groupvice4 theme
#5- Replaced deprecated hooks

These changes have been incorporated into version 3 (see top post)

(Actually I kind of merged my internal version that has support for 0.6-beta with this version to get the new version.)
