Cannot create indentity

User that login to RC the first time cannot create Identities. It has to logout and login and then can add identities.

Error log:

Once i'll try to clean up all and config all over again. I don't have to many new users.

Do you use MySQL?

Have you tried to resetup all tables using the SQL Dump?
